Key Responsibilities

For Home Healthcare Nurses:
• Conduct comprehensive health assessments and develop individualized care plans.
• Administer medications and treatments as prescribed and monitor clients’ vital signs.
• Provide education and support to clients and their families regarding health conditions and care procedures.
• Manage care transitions and coordination between different care providers.
• Document all assessments, interventions, and clients' progress accurately.

For Home Healthcare Social Worker:

• Assess clients’ social, emotional, and environmental needs to inform care planning.
• Coordinate care with healthcare providers and community resources to ensure a holistic approach.
• Offer counseling and support to clients and their families to navigate challenges and improve their quality of life.
• Advocate for clients’ needs and preferences within the healthcare system.
• Maintain accurate and confidential records of client interactions and outcomes.

Qualifications

For Home Healthcare Nurses:
• Current nursing license (RN or LPN) in the state of Ohio.
• Previous nursing experience is preferred but not required; recent graduates are welcome.

For Home Healthcare Social Worker:
• Current social work license in the state of Ohio.
• Strong knowledge of community resources and support systems for older adults.

General Qualifications for Both Roles:
• Compassionate, patient-centered approach with excellent communication and interpersonal skills.
• Ability to work independently and effectively within a team environment.
• Valid driver’s license and reliable transportation.
