To become certified by the Ohio Department of Aging, you must complete an online application. Refer to the Ohio Department of Aging Provider Certification Section for detailed instructions.
Before you can provide services, the participant must apply for and be approved for waiver services.
As a prospective provider, you must:
1. Complete the Ohio Department of Aging certification process.
2. Once certification has been approved, enroll both you and the participant with the financial management service (FMS).
3. Ensure the participant's case manager sets a service start date in their PASSPORT service plan.
Once these steps have been completed, you can begin offering services.
Long-Term Care Agency Provider: The provider is a legally organized entity that employs staff, is registered with the Ohio Secretary of State.
Non-Agency Provider: The provider is a legally organized entity owned and controlled by one person and does not employ staff.
Participant-Directed Service Provider: The provider is a person that a consumer directly employs and supervises to furnish a choice home care attendant service (CHCAS).
Assisted Living Waiver Provider: The provider shall be licensed as a residential care facility by the Ohio Department of Health.
